The $100 quarterly contribution for the April 1, 2012 Benefit Period under the Equity-League Health Trust Fund has a due date of March 1, 2012. If we receive your payment after March 15, 2012, we cannot assure that your eligibility records will be updated in time for the April 1, 2012 coverage. In the event that payments are received after March 31, 2012, you will incur two penalties - one being a $100 late fee and the other being that your coverage will not begin until your $100 contribution and penalty payments are made. You will also have only up to 31 days after the start of the April 1, 2012 quarter, which is through May 1, 2012.
