1) Use this form in order to rollover funds from another retirement plan into the Equity-League 401(k) Plan.

2) Fill out the form and attach the check for the amount of the rollover contribution. The check must be made out to MassMutual. If you are a non-participant, please pay special attention to the Investment Selection and Beneficiary Designation sections of the form. Please note that a "non-participant" is someone who has a 401(k) Plan with the Equity-League but is not currently employed under a contract that participates in the 401(k) Plan.

3) Sign above the line labeled "Participant" and fill in the date.

4) Please mail the completed form along with your rollover contribution to:
